Per the Times, Osbourne's certificate officially said that the icon died of cardiac arrest, acute myocardial infarction, coronary artery disease and Parkinson's disease with autonomic dysfunction. Osbourne died just weeks after he performed his Back to the Beginning Farewell show in his hometown of Birmingham, U.K. That show featured a reunion of Black Sabbath and performances from fellow hard-rock legends like Metallica and Guns N' Roses, raising about $190 million for charity. Osbourne performed one last time that day from a large black throne, singing classics like "Crazy Train," "Mama, I'm Coming Home" solo, and "War Pigs," "Iron Man" and "Paranoid" with Black Sabbath.
